Floorcovering company cited for ‘serious’ violation following September death at North Georgia facility

CHATSWORTH, Ga. — A penalty of $12,934 was proposed by the Occupational Safety and Health Administration after OSHA found Mohawk Industries committed a “serious” violation during an investigation into the death of an employee in September of last year at Mohawk’s facility at 1149 Duvall Road in Chatsworth. On Tuesday, an employee at the facility died from injuries sustained after being struck by a large roller. OSHA is investigating that incident as well.
Mohawk was notified on Feb. 28 of the proposed penalty relating to the Sept. 1 death of 42-year-old Vickie Dyer of Chatsworth.
Roy Glenn Bass, 59, of Crandall, died Tuesday at Murray Medical Center as a result of his injuries.
Messages left for Mohawk officials on Wednesday were not immediately returned. The company released a statement on Tuesday saying the cause of the accident involving Bass “remains unknown at this time.”
In the citation to Mohawk concerning inspections of the facility after Dyer’s death, OSHA wrote of a carpet roll up area, “On or about Sept. 2, 2017, and at times prior, the employer exposed employees to caught in between hazards, in that machine guarding was not provided to employees while working near rotating parts.”
The citation said Mohawk must provide “methods of machine guarding” to “protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are barrier guards, two-hand tripping devices, electronic safety devices, etc.”
The citation said the violation was “corrected during inspection.”
